Kentucky
Physicians Care Program
The Kentucky Physicians
Care Program consists of more than 3,000
physicians across the state who voluntarily
agree to accept patient referrals from eligible
patients. They treat these patients at no
charge. More than 500 primary care physicians
participate and every specialty is represented.
The Kentucky Physicians Care Program was
established in 1984 and is co-sponsored
by the Kentucky Medical Association.
Kentucky Pharmacy
Providers Program
The Kentucky Pharmacy
Providers Program combines products from
pharmaceutical companies with patient education
provided by pharmacists. More than 530 pharmacies
across Kentucky fill prescriptions for eligible
patients at no cost. Abbott Laboratories,
Bristol-Myers Squibb Company, Eli Lilly
and Company, Johnson & Johnson, Merck,
Novo Nordisk Pharmaceuticals, Pfizer and
Pharmacia Corporation donate the medications.
The Kentucky Pharmacy Providers Program
was established in July 1990 and is co-sponsored
by the Kentucky Pharmacists Association.
Kentucky
Home Health Care Program
The Kentucky Home
Health Care Program provides home-based
skilled nursing services to qualified patients
who are essentially home bound and require
intermittent skilled nursing services. A
home health agency makes an assessment visit
based on a referral from a participating
physician. There can be two to five additional
nursing visits following the initial assessment.
All 47 of Kentucky's home health agencies
participate. The Kentucky Home Health Care
Program was established in October 1992
and is co-sponsored by the Kentucky Home
Health Association.
Kentucky

of Hospices Program provides services to
eligible patients who are terminally ill.
All 29 hospice agencies in Kentucky participate.
The program began in 1992 and is co-sponsored
by the Kentucky Association of Hospices.
